Our caregivers communicate frequently with our Care Management team to ensure continuous care as our clients’ needs change.
To properly identify the need at the outset, our Care Manager conducts a personal assessment visit. This confidential process can include the individual, family members, and the appropriate medical team. Together, we determine the initial plan of care. Then, the most appropriate caregiver from our Reliable Caregivers team is identified and matched to the client.
Finally, a Care Manager carefully monitors the case as it progresses, making regular personal visits to ensure that the best possible care is provided at all times.
